Psychiatric Rehabilitation Services
The Psychiatric Rehabilitation Program is designed to assist individuals diagnosed with serious and persistent mental illnesses achieve fulfilling and successful lives.
RESIDENTIAL SERVICES
The three residential programs listed below are all located in St. Helens, Oregon. They are transitional living opportunities for individuals designed to promote skill development and community integration.
Ø Cornerstone is a licensed, sixteen-bed Residential Treatment Facility. This facility is staffed 24 hours a day, seven days per week. Cornerstone is frequently the entry point of residential services for individuals being discharged from State Hospital facilities. Residents have the opportunity to participate in a highly structured program that focuses on skill development, community integration and safety, medication and health management, and recovery goal development.
Ø Our House is a licensed, five-bed Adult Foster Home located in St. Helens Oregon. This facility is staffed 24 hours a day, seven days per week. Our House is frequently used as a step-down program for Cornerstone. Residents living at this home continue to work on developing the skills needed to move to a fully independent setting.
Ø Supported Housing Apartments is a semi-independent living opportunity for residents transitioning out of Cornerstone or Our House. Individuals live with a roommate in their own apartment while developing the final skills needed to move into unsupervised community setting. Assistance with medications monitoring and other independent living skills is available to those who need it by an on-site apartment manager.
PROGRAM DESIGN
Program participants living independently in Columbia County or at one of the residential options listed have access to the program options offered in the Psychiatric Rehabilitation Department. Program design is based on the Recovery philosophy of treatment which focuses on individualized, participant-centered treatment goals and supports for achieving those goals. There are three levels of services offered.
Ø Level I - Assertive Community Treatment (ACT) is available for qualifying individuals that have had recent hospitalizations or legal involvement due to their illness. Individuals participating in this program receive a high level of service designed to stabilize community tenure and develop/implement recovery-based goals.
Ø Level II – Illness Management and Recovery services are available for individuals who are interested in developing, implementing and achieving recovery-based goals. Participants have access to a wide variety of services and supports offered both in office and community settings.
Ø Level III – Recovery Support services are available for individuals that are living independently in the community with only occasional service needs.

Ø Group therapy and skills training
Ø Health management and exercise groups
Ø Housing placement assistance
Ø Free or reduced cost medication assistance through pharmaceutical programs for qualifying individuals
